Physical Requirements
This position requires intermittently sitting at a computer workstation for extended periods of time; and performing tasks with repetitive motions (such as typing); and occasionally standing or walking with objects weighing up to 25 pounds; and lifting, pushing, and/or pulling objects weighing up to 50 pounds. Also involves some exposure to injury from hazardous chemicals and fumes including waste, cuts/punctures, and blood-borne or airborne pathogens.
Shift
Primarily 3rd shift; 8:00 p.m. – 8:00 a.m. (7days on/7 days off). Includes some weekend and holiday requirements.
Job Summary
1. Support the Mission, Vision and Values of Pharmacy Services at UKHC.
2. Reviews medication lists daily for therapeutic duplication, omissions, dosing errors, drug interactions, and allergies.
3. Works collaboratively with providers to implement and communicate comprehensive medication evaluation and management for patients including between care settings.
4. Proactively prepares for and actively participates in reviewing patient profile, providing pharmacotherapeutic consults, making patient specific drug therapy recommendations.
5. Documents clinical activities pursuant to departmental expectations.
6. Reviews, interprets, and verifies new medication orders to ensure appropriate drug, dose, route, and frequency for adult and pediatric patients.
7. Collaborates with multidisciplinary team to ensure appropriate medication access, including recommending alternative therapies as appropriate.
8. Provides supervision of technical staff and confirms the quality and accuracy of dispensed products.
9. Maintains inventory and records on narcotics and other forms of drugs.
10. Compounds and dispenses medications upon receiving written (i.e. electronic) orders from physicians.
Skills / Knowledge / Abilities
Strong clinical skills; knowledge of inpatient drug distribution and dispensing; ability to prioritize; excellent communication skills; a team mentality; desire to teach; and motivation are required to excel in this position.

Preferred Education/Experience
PharmD and ASHP PGY1 residency and/or PGY2 specialty residency preferred.
