Summary: The Certified Occupational Therapy Assistant (COTA) delivers skilled occupational therapy services under the supervision of a licensed Occupational Therapist (OT). This role focuses on implementing treatment plans, supporting patient independence, and promoting safe, functional living in the home environment.

Essential Functions:

  • Implements OT interventions per the stablished plan of care.
  • Support patients with ADLs/IADLs/ mobility, transfers and home safety.
  • Lead exercises for strength, coordination and range of motion.
  • Train patients on adaptive equipment and reinforce home programs.
  • Observe patient progress and response to treatment.
  • Report changes in condition, safety concerns or functional status to the supervising OT.
  • Assist with data collection for care planning.
  • Educate patients and caregivers on exercises, equipment use, and safety strategies.
  • Promote adherence and engagement in the plan of care.
  • Maintain timely, accurate EMR documentation.
  • Follow infection control, safety and Agency guidelines.
  • Participate in trainings, supervision and quality improvement initiatives.

Requirements:
  • Active and unrestricted COTA license from an accredited OTA program required.
  • 1+ year experience as a COTA preferred.
  • Valid CPR certification, driver's license, reliable transportation and insurance required.
  • Strong communication, clinical judgement and EMR skills.
  • Ability to follow care plans and work independently.
  • Maintain HIPAA compliance and professionalism at all times.
  • Complete visits and documentation accurately and on time.
  • Participate in trainings and uphold Frontier Home Health's standards.
  • Support patients with ADLs/IADLs, mobility, transfers and home safety.
  • Lead exercises for strength, coordination and range of motion.
  • Train patients on adaptive equipment and reinforce home programs.

Physical and Environmental Demands:

  • Prolonged standing, walking, bending and reaching required.
  • Ability to lift, push and move up to 50 lbs.
  • Manual dexterity and hand-eye coordination required.
  • Ability to work under time constraints and varied home environments.
  • Ability to communicate effectively in English, both verbally and in writing. Communication in additional languages is preferred.
