Job Title: Clinician Navigator
Department: Interventional Services
Location: Unity Hospital
Hours Per Week: Full Time - 40 Hours
Schedule: Monday through Friday - Days; No weekends, holidays, or on-call.
SUMMARY
As a Clinician Navigator, your experience and understanding of chronic illness is key in advocating for patients and providing them with the best healthcare treatment resources and optimal clinical outcomes.
IR/Cath Lab
The Cardiology Lab (Cath Lab) and the Interventional Radiology Lab are high-acuity procedural areas serving a diverse adult patient population. The RNs on this unit will support adult patients, often with comorbidities such as coronary artery disease, stroke risk, peripheral vascular disease, etc. You will collaborate with ICU, ED, and inpatient units for urgent/emergent cases. We use advanced imaging systems (e.g., fluoroscopy, CT-guided interventions), hemodynamic monitoring, and sedation protocols. This role requires adaptability, strong clinical judgment, and the ability to work in a fast-paced, procedure-driven environment with frequent transitions between patient care and technical support.
ATTRIBUTES
Five (5) years of acute hospital care experience
Excellent oral and written communication and interpersonal skills
Strong problem solving, decision making and critical thinking skills
Ability to recognize and manage emergencies
Strong knowledge of specific chronic care regimen
Time management and organizational skills
Must be self-directed and able to collaborate
RESPONSIBILITIES
Patient Care. Serve as the patient’s first line of communication throughout the entire treatment program; coordinate multiple and complex technologies as part of the diagnosis and treatment; coordinate appointments and procedures; perform direct patient care, nursing documentation, symptom management, patient and family education and counseling
Leadership. Form a collaborative practice among the care management team (i.e. surgeons, nurses, social workers, etc.); provide community outreach and education regarding the program; respond to calls concerning the program
Collaboration. Work closely with the physician during patient visits; become aware of the treatment plan, expected outcomes and possible treatment complications

PHYSICAL REQUIREMENTS: M - Medium Work - Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects; Requires frequent walking, standing or squatting.
LICENSES/CERTIFICATIONS:
- RN - Registered Nurse - New York State Education Department (NYSED)
- BLS - Basic Life Support - American Heart Association (AHA)
- NP - Nurse Practitioner Certification - New York State Education Department (NYSED)
- OT - Occupational Therapist - New York State Education Department (NYSED)
- PT - Physical Therapist - New York State Education Department (NYSED)
PAY RANGE: $82,742.00 - $113,568.00
The listed base pay range is a good faith representation of current potential base pay for successful applicants. It may be modified in the future. Pay is determined by factors including experience, clinical licensure date, relevant qualifications, specialty, internal equity, location, and contracts.
