About This Position

JOB SUMMARY:
As a Case Manager Supervisor you will supervise a minimum of five case managers, meet with case managers at least once weekly or as needed, perform audits on case files regularly, lead audit reviews with county representatives and DHS, complete quarterly unit reviews, manage time off request and time cards, act as a main point of contact for county liaisons, provide support for case managers on a daily basis, complete performance reviews and correct action plans, engage in community outreach, and implement the agency’s racial equity plan. These duties will include culturally specific case management services with the African American, African, Native American, Asian, and Hispanic communities.

Core Responsibilities
Interview potential new hires
Evaluate employees’ performance
Coordinate and facilitate use of departmental and community service resources and make appropriate referrals
Review employee’s documentation/case notes/CSSPs
Monitor employees progress goals to meet their individual needs
Consult with departmental staff, other county departments and community service professionals to develop and provide appropriate and timely resources for employees
Provide crisis intervention for employees
Investigate complaints of employees’ performance
Engage and provide outreach to the community regarding the agency and waiver services
Implement racial equity strategies and culturally specific case management strategies for clientele and the community.
Develop techniques and resource materials for employees
Provide consultation to employees
Represent the unit/department in committee assignments and interactions with community groups for service provision and program planning
Develop and provide training to employees and service providers.
Review employee’s case records and complete forms for documentation of service referrals, evaluation of individual outcomes and general case management purposes.
Monitor employees’ compliance with individual services
Provide data used to monitor trends and developments relating to individual and program needs
Provide problem solving consultation to employees, county teams, service providers, and other agencies.
Be up to date on DHS and County changes and updates on program
Writing, organizing & managing program procedures & policies: Reviewing and monitoring if procedures/guidelines are being followed by employees
Monitor CDCS cases’ budgets and other required documents
Assign, monitor and close incoming/outgoing of case files
Coordinate/Plan coverage for employees on vacation
Time management/Billables tracking
Track initial meetings, annual meetings, CSSP dates, 65th birthdays, 6 months meetings, and annual reassessments (6791E) for all cases.
Conducting monthly case audits as required by Lead Agency
Assuring the integrity of all Case Management waiver billing in that the services delivered must have prior authorization and meet required waiver service definitions and must be delivered before billing can occur
Maintain the relationship with Lead County
Complete Quarterly Reports for Lead County
Adhere to Lead County expectations & duties
Other duties as assigned

REQUIREMENTS:
4-year degree in Social Work and Licensed Social Worker by the MN Board of Social Work or related Bachelor’s degree.
Must have at least two years of full-time social service experience in the education or treatment of persons from vulnerable populations
Experience in Elderly Waivers, AC, DD, CADI, BI, and CAC required.
Knowledge of MA, Medicare, and MN Healthcare Programs strongly preferred.
Experience and knowledge of working with the following communities: black/African-American, East and West African, American-Indian, Hmong, Karen, and Latino required
Knowledge of historical racial trauma, cultural responsiveness, restorative practices required.
Background clearance
Ability to effectively communicate in English, written and verbally.
Driver's license, reliable vehicle you can use for work purposes, acceptable driving record and proof of auto insurance required.
Willing to drive own vehicle to perform job duties.
Must be at least 21 years of age.
Live in the State of Minnesota.

Pay: $28.00 - $32.00/hr Based on Experience
