LPN - Full Time at Salem Community Corp – Worcester, Massachusetts

The Elms Assisted Living at The Briarwood Community is seeking a compassionate and skilled Wellness Nurse (LPN) to join our care team. This full-time role offers flexible scheduling and a competitive hourly rate of $32.00–$38.50, providing an excellent opportunity for an LPN committed to delivering high-quality, resident-centered care. Every other Weekend required.

• Provide nursing care and support to residents in accordance with individualized care plans.

• Administer medications and treatments following physician orders and facility protocols.

• Monitor resident health status, report changes promptly and assist with assessments.

• Collaborate with RNs, CNAs, physicians, therapists, and families to ensure coordinated care.

• Maintain accurate documentation, medication records, and progress notes.

• Support wellness initiatives, education, and preventive care practices within the community.

• Current Massachusetts Licensed Practical Nurse (LPN) license in good standing.

• Strong clinical, organizational, and communication skills.

• Ability to work effectively with residents, families, and interdisciplinary team members.

• Experience in assisted living, long-term care, wellness programs, or skilled nursing preferred.

• Commitment to providing compassionate, person-centered care.
