They do this by evaluating the psychosocial issues of patients to ensure transition planning, crisis management, and planning/coordination of both internal and external services. Responsibilities

The Resource Specialist will provide Care Management services to patients who have been identified by their primary care provider or staff members at Legacy Clinics as needing ancillary mental health / psychosocial services. Key services include (but are not limited to) psychosocial assessment; facilitating access to community resources; coordination with community organizations on behalf of patient; crisis management; access to medications and insurance; transition planning; education. Works as part of multidisciplinary team which may include physicians, pharmacists, clinic providers and staff, and other Care Management staff. Refers patients and families to the Clinic Social Workers in complex cases which are both clinical and psychological in nature. Examples might be AIDS, complex family issues, difficult abuse/domestic violence cases, and dual diagnosis cases.



Qualifications

Education/Experience:

Bachelor's Degree or any combination of equivalent education and case management experience.

Experience in community-based case management

Experience in assessment and care plan development.

Skills:

Strong assessment skills and ability to assess urgency of need, and triage for services

Knowledge of community resources available to patients, caregivers and families

Knowledge of the community social service delivery system.

Ability to integrate knowledge of the normal aging process, changes related to chronic illness, and needs of patients with chronic illness.

Excellent organization, oral, and written communication skills for effective interaction with patients, patient’s families, physicians and staff

Strong advocacy skills

Computer ability and knowledge for entering and retrieving patient data, preparing letters and summary information

Ability to foster communication and build relationships among Care Management team members, providers and clinic staff

Excellent time management skills

Ability to read and interpret medical records
