About Position: The ultimate aim of the Encore 49 Residence is to prevent recidivism of hospitalization and/ or homelessness and improve quality of life for older New Yorkers. The Residential Psych Nurse is responsible for promoting clinical excellence and evidence-based practice. The Residential Psych Nurse supports a healthy work environment in which a team-based, patient centered care can flourish. The Residential Psychiatric Nurse will be a part Encore 49’s residential team providing clinical direction and support. They will report to the Senior Director, while collaborating with the team consisting of the Residential Manager, Residential Coordinator, Case Managers, Psychiatrist, License Social Worker, Visiting Nurses, and Home Health Aides.

Requirements:

Responsibilities:

  • Conducts medical health assessments, behavioral health pre and post evaluations, care stabilization, and referral services to clients in need of follow up appointments
  • Work with the Residential Coordinator to update and maintain medication advocacy; ensuring all medication changes are documented and followed.
  • Using a client-centered approach to client care, the Psychiatric Nurse Practitioner will ensure that the clients receive an integrated, multidisciplinary assessment and immediate stabilization plan.
  • Assessments are to include client choice and/or mandated care, harm reduction, trauma-informed care, and psychosocial rehabilitation.
  • The Residential Psychiatric Nurse will perform with a demonstrated ability and sensitivity providing services to highly vulnerable individuals who have history of homelessness, substance affected, and/or have serious and persistent mental illness.
  • The Residential Psychiatric Nurse will work with culturally and lifestyle diverse populations.
  • Refer individuals to immediate/urgent care as well as establish community connections with primary care, mental health services and substance use disorder care.
  • Provide preliminary health diagnosis, care, and treatment plans to clients.
  • Provide crisis intervention assistance when needed.
  • Document each client interaction, health data and notes in the AWARDS system.
  • Coordinate and follow-up on internal and external doctors’ appointments, hospital and rehabilitation admissions of clients including healthcare assessments as needed
  • Collaborate with the Residential Manager, Buildings and IT Manager and other interdisciplinary staff to prioritize placement meetings for Encore 49.
  • Establish and maintain community health resources with direct linkages to care.
  • Participate in internal and external health care management meetings with Senior Director, Program Director, Case Managers, and other care coordinators when needed.
  • Administer medication as prescribed when needed, including but not limited to ointments, injections, ophthalmic, etc.
  • Manage and report quarterly on any an assessment and treatment outcomes.
  • Maintain a safe and confidential working environment that meets federal, state, funding source and Encore Community Services requirements.
  • Actively participate in the weekly Case Review Meeting along with the monthly Psych meetings with the Psychiatrist.
  • Create, update, and add addendums to current medical Policy and Procedures, ensuring adherence to state, federal, and Encore 49 requirements and regulations.
  • Clearly and accurately document all incidents and report in a timely manner
  • Ensure timely reporting, documentation and record-keeping and hard copy files in accordance as mandated by funding sources or agency administrator, in an orderly and up-to-date manner;
  • Actively participate in the interdisciplinary team approach at Encore 49, including communicating with the Senior Director, Residential Manager and Buildings and IT Manager.
  • Participate in staff meetings and supervisory sessions.? All other duties as assigned by supervisor(s).

Required Qualifications:

  • Sensitivity and empathy for all residents;
  • Current Registered Nurse Licensure in NY State
  • Psychiatric Mental Health Nurse Certification
  • Active Medicare/Medicaid Registration preferred
  • Commitment to providing care services for individuals experiencing homelessness, mental health disorders, and/or substance use.
  • Demonstrated experience working with diverse educational, lifestyle, ethnic and cultural origins
  • Excellent interpersonal skills and ability to work as both part of a team and independently
  • Strong written and verbal communication skills
  • A proactive person-centered approach
  • Ability to triage emergencies when needed (can be via telephone
  • Flexibility in schedule required
  • Commitment to Encore’s mission and to uplifting the voices of people with lived experiences of homelessness and mental health disorders
  • Desire to perform the essential functions of this job
  • Ability to handle emergency situations

Preferred Qualifications:

  • Proven experience with the seniors who are formerly homeless, and/ or those with mental health diagnoses;
  • Expert written, oral, and interpersonal communication skills;
  • Experience working both independently and in a team-oriented and collaborative environment;
  • Competency in using relevant patient care data base systems preferably AWARDS;
  • Excellent organization skills;
  • Works well under supervision;
  • Works well and takes initiative in a fast paced, high intensity and ever evolving environment;
  • Bilingual a plus.
