About the Role:
The Care Partner CHHA (Certified Home Health Aide) plays a vital role in providing compassionate and personalized care to individuals in their homes, ensuring their comfort, safety, and well-being. This position focuses on assisting clients with daily living activities, including personal hygiene, mobility, and medication reminders, while fostering a supportive and respectful environment. The Care Partner CHHA collaborates closely with healthcare professionals and family members to implement care plans tailored to each client's unique needs. Through attentive observation and communication, the Care Partner identifies changes in clients' conditions and reports them promptly to the appropriate medical personnel. Ultimately, this role contributes significantly to enhancing the quality of life for clients by promoting independence and dignity in a home setting.
Minimum Qualifications:
- Valid certification as a Home Health Aide (HHA) or Certified Home Health Aide (CHHA) as required by the State of New Jersey
- High school diploma or equivalent.
- Current CPR and First Aid certification.
- Ability to pass background checks and health screenings as mandated by the employer.
- Demonstrated ability to perform physical tasks associated with caregiving, including lifting and transferring clients.
Preferred Qualifications:
- Previous experience working as a Home Health Aide or in a similar caregiving role.
- Additional certifications related to elder care or chronic disease management.
- Familiarity with electronic health record (EHR) systems for documentation purposes.
- Strong communication skills in multiple languages to better serve diverse client populations.
- Training in dementia care or palliative care.
Responsibilities:
- Assist clients with activities of daily living such as bathing, dressing, grooming, and toileting to maintain personal hygiene and comfort.
- Support clients with mobility needs, including transferring, walking, and positioning to prevent falls and promote physical well-being.
- Provide medication reminders and assist with simple health monitoring tasks as directed by healthcare professionals.
- Prepare and serve nutritious meals according to dietary guidelines and assist with feeding when necessary.
- Maintain a clean, safe, and organized living environment to support clients' health and safety.
- Observe and document clients' physical and emotional conditions, reporting any significant changes to nursing staff or supervisors.
- Offer companionship and emotional support to reduce feelings of isolation and promote mental well-being.
- Adhere to all safety protocols, infection control procedures, and confidentiality standards in accordance with regulatory requirements.

The Care Partner CHHA utilizes strong interpersonal and communication skills daily to build trust and rapport with clients and their families, ensuring clear understanding of care plans and client needs. Physical stamina and dexterity are essential for safely assisting clients with mobility and personal care tasks.
