Essential Responsibilities:

1. Assess patients' needs and develop individualized care plans

2. Coordinate and schedule medical appointments, procedures, and tests

3. Collaborate with physicians, nurse practitioners, and healthcare professionals to streamline care

4. Educate patients and families about their conditions, treatment options, and medications alining with the providers' assessment and plan

5. Closely monitor patients' progress

6. Maintain accurate and up-to-date patient records

7. Participate in quality improvement activities

Qualifications:

1. Must live in CT
2. Possess advanced expertise in handling prior authorizations
3. Knowledge of healthcare regulations and standards
4. Minimum of 2 years of experience in a healthcare setting
5. Strong organizational and time management skills
6. Excellent communication and interpersonal skills
7. Ability to work independently and as part of a team
8. Ability to think critically and problem-solve
9. Strong attention to detail and ability to multitask
10. Possession of a Health Coaching certificate is viewed as an added advantage.
