About This Position

Title: Therapist - Care Coordinator

Department: Community Specialty Services

Shift: FT/ Schedule will vary

Location: 4730 N Winthrop and offsite.

We are seeking a highly motivated and experienced Licensed Clinician to join our team. The ideal candidate will have a strong background in providing individual and group therapy, and be passionate about helping individuals achieve their goals. This role will involve providing a variety of clinical services, including crisis intervention, counseling, and advocacy.

Heartland Alliance Health’s (HAH) mission is to transform healthcare for the most vulnerable – particularly people experiencing displacement such as refugees and immigrants from diverse cultural backgrounds.

Will complete the requited online HAH training within the first week of work. Will learn to document in Athena within one week of the training and will have three weeks after that to get up to speed with the below expectations.

  1. Individual therapy – a minimum of 15 sessions per week
  2. Group therapy – a minimum of 2 groups per week
  3. Documentation – all participant interactions will be documented and billed in Athena within 72 hours of the service being provided
  4. Develops therapeutic relationships with participants through the use of Motivational Interviewing, care that is Trauma Informed and from a Harm Reduction framework.
  5. This role involves delivering culturally sensitive therapy and emotional support, focusing on addressing the unique experiences and challenges faced by refugee and immigrant populations in a non-residential setting.
  6. The therapist will provide culturally sensitive and trauma-informed care to refugees and immigrants, utilizing interpreters as needed.
  7. Completes bio-psycho-social assessments of participants as needed
  8. Develops treatment plans with participants as needed
  9. Provides intense advocacy, crisis intervention, counseling and problem solving in a variety of areas for all clients
  10. Develops and maintains relationships with outside providers, case managers and hospitals.
  11. Consults with the Psychiatrist regarding treatment reviews.
  12. Adheres to HIPAA privacy standards and the ethical guidelines of a licensed clinician
  13. Consults with other staff members about program participants and participates in clinical cases discussions
  14. Attends staff meetings as required
  15. Maintains therapist license.
  16. Participates and contributes in regular supervision meetings, team meetings, in house training sessions and conferences
  17. Makes referrals in accordance with service plans to other Organization units and external providers.
  18. Works cooperatively with clinical case managers and primary and psychiatric services at the HAH clinic to maximize service outcomes.

Position Requirements

  1. Have an unrestricted license (LSW/LCSW or LPC/LCPC)
  2. Be able to work in the US without sponsorship
  3. Pass a background check and credentialing
  4. Be vaccinated against COVID-19 and the Flu as well as be up to date on other required vaccines (exception can be made for health or religious reasons – ask supervisor if details are needed)
  5. Fluent in second languages - highly preferred.
