Social Worker - ACHS at AcuteCare Health System – Lakewood Township, New Jersey

Position Summary: The Social Worker will provide Social Work services to patients and families at the Specialty Hospital of Central Jersey, serving high-acuity post-ICU patients.

The social worker will collaborate with the Administrative team, Case Management, Physicians, and Nursing staff to meet the needs of the patients and their families.

The Social Worker is responsible for Family meetings, Assessments, Discharge planning, and emotional support for patients and families.

Required: Master’s Level Social Worker- Licensure by The New Jersey State Board of Social Work Examiners. LSW/LCSW, CSW

Experience: One to three years prior experience in social work is preferred. Hospital experience is a BIG plus!

Skills: Computer literacy, Effective verbal and written communication skills. Familiarity with the psycho-social issues of the frail, chronically ill and their caregivers.
