Social Work/Case Manager (MSW-StayWell Senior Care)

Responsible for direct social work case management services to the participants of the StayWell Senior Care program. Provides social services support as: participant screening, case management, counseling, and referral. Master's degree from an accredited School of Social Work. Possess individual therapy skills for geriatric participants, analytical thinking and problem solving abilities. Willing to conduct home visits to private homes of participants in Randolph, Montgomery and Moore counties. Only act within the scope of authority to practice. Meet standardized set of competencies for the specific position description established by StayWell Senior Care and approved by CMS before working independently. Have valid NC driver's license and vehicle. Minimum two (2) years' experience in a health related area. Minimum one year experience working with the frail elderly population.
