Associate Care Navigator-Philadelphia, Part Time

This is Part Time, (Tuesday and Thursdays)


The Associate Care Navigator works with clients and care-givers/families to provide service coordination and accompaniment and transportation to medical appointments. The Associate Care Navigator works under the guidance of the Care Navigator and in collaboration with the Care Navigation and Entitlement team.

Duties and Responsibilities:
  1. Provides transportation and accompaniment to medical appointments, as necessary
  2. Accompanies client to ER, when necessary
  3. Collaborates with Care Navigation team to meet Lifeplan goals of client
  4. Attends discharge and care planning meetings as client advocate and documents any changes for client and Care Navigator
  5. Plans and coordinates all aspects of client moves
  6. Coordinates and arranges for personal care items as requested by client and approved by Care Navigator
  7. Coordinates external services as requested by client and Care Navigator
  8. Provides emotional support and companionship to clients as needed
  9. Reports any change of client status to Care Navigator and client caregivers
  10. Documents all interactions and occurrences in Home Health Exchange.
Educates and Advocates for the client in all venues. Functions as client’s health care advocate.

Specialized Skills and Competencies:
  1. Bachelors degree required; can be in process of obtaining degree. Nursing students are welcome to apply. Candidate will receive on the job training.
  2. Candidate must have valid Pennslyvania driving license and be willing to drive clients.
  3. Candidate will demonstrate independence, flexibility, responsiveness and good organizational skills
  4. Understanding and ability to deliver highest degree of customer focused services.
  5. Excellent verbal and written communication skills.
  6. Hours are 9am to 1pm on Tuesday and Thursdays and college schedule can be accommodated. Must be available to travel within Philadephia PA and surrounding areas.
