Outreach Representative at ZUFALL HEALTH CENTER INC – Morristown, New Jersey

Under the direction of the VP of Community Engagement, the Outreach Representative will identify individuals and families in need of health care services, family planning education and link them to ZHC. Outreach will mainly be conducted through Community Based Organizations, churches, schools, local government, and non-government agencies to increase awareness of Zufall health services and increase referrals to Zufall.

Essential Functions, Duties and Responsibilities

• Assists in obtaining essential support services such as Medicaid, food stamps, transportation, translation services and day care
• Providing cancer education awareness/CEED services throughout Somerset County
• Serves as an advocate for the patients, and acts as a liaison between the patients and ZHC
• Attends in-service education and training programs
• Provides basic health information and education to patients
• Arranges referrals and coordinates transportation as needed
• Assists medical staff in care management through outreach using patient registries
• Provides ongoing social and emotional support through care management
• Completes daily activity logs, referral forms and other reports, as necessary
• Provides sexual health education topics to students and other community members throughout ZH service area
• When presenting to schools, they must arrive between 7-7:30 AM and remain until about 2:30 PM
• Assists in monthly data scrubbing – searches for missing data in provider charts and corrects monthly reports for both programs
• Assists manager with other duties as time permits depending on school presentation schedule
• Assumes other program related tasks as assigned

Knowledge, Skills, and Abilities

• Ability to communicate in English, both orally and in writing; ability to speak Spanish fluently
• Ability to operate a motor vehicle and travel within the ZHC service site areas
• Strong interpersonal and communication skills, and the ability to demonstrate empathy
• Ability to relate to patients as well as to local government and community organization workers and leaders
• Ability to work independently, seeks assistance when necessary
• Works with medical and social work professionals in a multi-disciplinary team, and works in a fast-paced work environment
• Possesses basic knowledge of target populations’ ethnic and medical needs
• Possesses flexibility, cultural sensitivity, and tactfulness
• Maintains HIPAA compliance in all regulated transactions and complies with all regulatory standards
• Comfortable with public speaking
• Must be able to talk about sex, contraceptive methods, sexually transmitted infections, sexual/reproductive/pregnancy coercion- Training will be provided on these topics.

Education, Training, and Experience

• High School diploma or Equivalent
• Bilingual in English and Spanish Required
• Comfortable with Public Speaking, topics regarding sex, STIs, and pregnancy coercion required
• Previous experience in Outreach preferred
