Home Health Speech Therapist (ST) at One Home Health LLC – Merrillville, Indiana

We are seeking a Part-Time/PRN Speech Therapist (ST) to join our home health team serving Lake County, IN. • Perform in-home speech-language evaluations and skilled treatments
• Assess and treat speech, language, voice, fluency, and swallowing disorders
• Develop and implement individualized plans of care
• Educate patients, families, and caregivers on treatment strategies
• provide care plan guidance to interdisciplinary team members
• Refer to other disciplines as appropriate and document accordingly
• Supervise HHAs when services relate to the speech-language plan of care
• Complete accurate, timely documentation in compliance with agency standards
• Participate in interdisciplinary collaboration as needed

• Licensed Speech Therapist in Indiana
• ASHA CCC-SLP or CFY candidates actively completing supervise experience
• Minimum 1 year of healthcare experience (home health preferred, not required)
• Strong clinical judgement and communication skills
• Ability to work independently in a home-based setting
• Current CPR certification
• Valid drivers license and insured vehicle
