PPSO provides more than 30,000 patient visits each year at four health centers and via telehealth. PPSO is also regarded as the region’s most respected provider of medically accurate sexuality education for young people and adults, as well as training programs for professionals who work with youth and families. Position Summary

The Follow-Up Coordinator is responsible for supporting the Centralized Follow-Up Team (CFT) in completing needed follow-up care by contacting patients, sending reminder letters, reconciling lab results, communicating with health center and Centralized Follow-Up Team members, and assisting with referral and records processes.

RESPONSIBILITIES:

Essential Duties

  • Contact patients (or attempt to contact patients) regarding normal lab results under the direction and follow-up plan set by licensed Centralized Follow-Up Team members.
  • Monitor incoming faxes and distribute to appropriate parties.
  • Enter non-interfaced (paper), and faxed lab results into patient electronic health record (EHR) charts per CFT protocol.
  • Reconcile lab tests ordered with lab results received and contact outside lab(s) as needed to receive results.
  • Reconcile open lab orders to ensure appropriate follow up in patient care.
  • Assist in medical referrals by communicating with patients, outside providers, and/or third-party payers, as well as sending or requesting medical records related to referrals.
  • Assist with Release of Information (ROI) and work closely with Compliance Team as needed.
  • Document and maintain meeting/huddle notes for CFT and assist in other document preparation as needed/directed.
  • Maintain ongoing communications with Health Center staff via EHR, telephone, Microsoft Teams, ZOOM, email, and other communication systems as necessary or directed by leadership to ensure continuity of patient care.
  • Ensure the integrity of the patient record by ensuring all information is recorded and accurate.
  • Maintain patient confidentiality and adhere to HIPAA guidelines and other applicable state and federal laws and regulations.
  • Assist in the continued improvement of systems to streamline patient follow-up processes and procedures.
  • Actively participates in the delivery of reproductive health services, including abortion care. Participation can include but is not limited to referrals and education.
  • In cooperation with Leadership, offer or develop evidence-based practice recommendations for improving patient care, health center efficiency, and quality customer service.
  • Attend all-staff, licensed staff, trainings, improvement meetings, huddles, debriefs and other meetings as scheduled, directed, or requested; virtual and in-person.
  • Participate in quality improvement initiatives and professional development opportunities to stay current with the latest trends and best practices.
  • Accept individual and joint responsibility for clinic efforts to comply with all organizational quality assurance guidelines and pertinent government regulations including OSHA, CLIA and HIPAA.
  • Establish an effective working relationship with internal and external customers, especially as part of a physically distanced team (team members working from multiple sites).
  • Notify the appropriate contact person when issues or problems arise.
  • Other duties as assigned.

QUALIFICATIONS AND EXPERIENCE:

Minimum Required Qualifications

  • High school diploma or equivalent.
  • At least one (1) years’ experience in the healthcare field.
  • Comprehensive understanding of medical terminology.

Preferred Qualifications

  • Bi-lingual in Spanish is preferred.
  • Proficient with computers and MS Suite (Excel, Word, etc.)
  • Familiarity with HIPAA policies and compliance.
  • Experience understanding and interpreting set policies and procedures.
  • Experience with EPIC or other EHR.
