Overview

The Legacy/OHSU Interventional Radiology practice provides services at four locations (EMC, GSMC, MHMC and MPMC) including ambulatory clinics at each site. The IR team cares for both adult and pediatric patients with a wide variety of diseases requiring image guided interventions and management. The RN Navigator is a key member of the multi-disciplinary patient care team, which includes physicians, advance practice providers, medical assistant, schedulers, and other clinical and management support staff. Responsibilities include indirect patient care, responsibilities related to EPIC, patient triage (includes telephone triage), participation in quality improvement, and program and staff development. This position requires independent critical thinking, very strong assessment skills, case management, transition of care, and the ability to appropriately triage patients with complex, conditions. The RN must be able to work as a team with providers, nurses, medical assistants, schedulers, and all other disciplines as needed, and functions in a collaborative manner to ensure effective and timely communication with the patient, family, and other team members.



Responsibilities

Clinical Practice: Participation in standards development. Defined patient outcomes are achieved through provision of expert information, recommendations and collaboration in care and clinical practice.

Serves as a clinical resource for patients/families and is identified as such by the clinical service area and physicians.

Collaborates with patients/families and members of the multidisciplinary care team in the development, implementation and evaluation of the patient Plan of Care.

Facilitates transition planning with patients/families and members of the multidisciplinary care team.

Streamlines patient continuum of care transitions and logistical issues, by providing liaison role between patient and medical staff, allied healthcare providers and other community resources.

Ensures care plan is current and is communicated to all care team members.

Provides assessment, teaching, communication, emotional and spiritual support with a holistic approach.

Provides education to patient/family in order to assist with patient-led treatment decisions.

Participates in problem-focused teams when needed.

Patient/family contact

Assesses patient medical, educational, emotional, social, financial and logistical needs.

Initiates patient contact at referral and high stress points (i.e., at diagnosis, and/or when symptom management issues arise) in the care continuum.

Collaborates with members of the interdisciplinary team, including physicians, clinic and hospital personnel.

Informs patient of and refers him/her on, as appropriate, to community resources, counseling and support groups.

Refers patient/family on to the appropriate resources to address transportation, medication, health insurance and financial issues.

Provides patient/family with resource contact information.

Facilitates scheduling of medical consultations and support services if appropriate.

Facilitates access between patient and physician(s).

Facilitates access to support staff as needed (i.e., social worker, dietician, pharmacist, therapist, etc.).

Education: Theories and principles of education are utilized to support the educational development of patients and families.

Assesses patients’ understanding of diagnosis, treatment options, plan of care, medical terminology, resources and clarifies information as needed.

Participates in the development and implementation of teaching materials.

Utilizes and/or develops/revises education materials.

Encourages patient to keep a log of questions to ask his/her healthcare provider(s) and suggests retaining copies of patient’s own medical records.

Encourages and/or assists patient to contact physician/allied healthcare staff for follow-up questions.

Assesses patient/family understanding of discharge instructions and follow-up care.

Community outreach and professional development activities

Organizes and facilitates support and educational groups specific to diagnosis.

Organizes, facilitates and presents professional/community education programs (i.e., prevention, screening, early detection, diagnosis, treatment, symptom management and survivorship).

Attends unit trainings/meetings and conferences as required.

Continually stays current with department staff RN educational/skills competencies.

Research studies

Actively identifies patients for LH research studies.

Assists in the development of LH research studies.

Assists with research activities.

Provides patient education regarding specific research studies, provides research brochures when appropriate, and makes patient referrals to research staff.

Quality/Care Services: Leadership is provided in the utilization of CQI principles to continuously improve and evaluate innovation for high risk, high volume patients and/or system variances.

Participates in and provides leadership for Continuous Quality Improvement (CQI) activities.

Identifies, develops and monitors program quality indicators relating to coordination of care, compliance with plan of care and patient satisfaction.

Utilizes quality indicators in ongoing program development.

Serves as ambassador, representing clinical service area.

Performs all duties in a manner which promotes team concept and which fulfills Legacy vision and mission.



Qualifications

Education:

Academic degree in nursing (BSN or MSN/MN) required; MSN/MN preferred.

Experience:

Minimum two years progressively more responsible clinical experience in the specialty field directly related to the assigned patient population.
