About This Position

About the Opportunity

This position is responsible for guiding youth on their path to independence through effective relationship building, support, supervision, counseling, assessment, service planning, coordination of services, ongoing evaluation of needs, and documentation of services provided.

As assigned, the Case Worker will perform these duties either in accordance with the Commonwealth of Virginia Department of Social Services Standards for Licensed Child-Placing Agencies through VHBG’s Fostering Futures Program, or within the Pride Place Program.

Core duties and responsibilities:

Supporting, Supervising, and Counseling Youth

  • Provide casework services for a designated caseload of youth
  • Building relationships with youth to facilitate effective collaboration in developing goals and achieving progress
  • Orienting youth to program
  • Supervising youth in a least restrictive manner
  • Supporting youth through on call, 24/7 phone access; in person if needed
  • Counseling youth in preparation for discharge

Assessment of Needs; Planning and Coordinating Services

  • Interviewing youth to determine needs, strengths, and abilities
  • Assessing suitability of living arrangement
  • Assessing need for psychosocial, nutritional, medical, dental, and educational services
  • Coordinating services to reduce barriers; linking youth with appropriate services
  • Ensuring services are provided
  • Collaborating with FAPT and other involved parties in plans, as applicable
  • Continually evaluating and reviewing the youth’s plan of care; updating and linking with services as needed

Documentation

  • Preparing and maintaining case records, as applicable for the program, including:
  • Social histories
  • Written agreement with youth
  • Service Plan
  • Social History
  • Face Sheet
  • Case Notes
  • Quarterly Progress Summaries
  • Serious Incident Reports
  • Discharge Summary

Other:

  • Responsible for attending all related training, meetings and special events as scheduled

Minimum Education and Experience:

  1. A doctorate or master's degree in social work from a college or university accredited by the Council on Social Work Education or a field related to social work such as sociology, psychology, special education, or counseling, with either a student placement in casework services to children and families or one year of experience in providing casework services to children and families;

OR

A baccalaureate degree in social work or a field related to social work including, but not limited to, sociology, psychology, special education, or counseling and one year of experience in providing casework services to children and families;

OR

A baccalaureate degree in any field plus two years’ experience in providing casework services to children and families.
