Location: Chicago, IL 60621
Pay: $22.20 - $26.64 per hour ($46,176 - $55,411 annually)
Schedule: Full Time, Non-Exempt (40 hours/week)
Work Arrangement: On-site (Hybrid eligible after 90 Days)

Program Overview: Support Services for Veteran Families (SSVF) is a comprehensive housing stability program designed to support Veterans who are experiencing homelessness or are at risk of losing stable housing. The program focuses on helping Veterans quickly secure safe, stable housing while providing the supportive services needed to maintain long-term independence and stability.
We provide:
  • Housing navigation and placement assistance to help Veterans locate safe, affordable housing
  • Personalized service plans developed in collaboration with Veteran Services staff
  • Connections to community resources including healthcare, employment services, and financial assistance
  • Landlord engagement and housing advocacy to expand housing opportunities for Veterans
  • Housing stability support including case management, referrals, and ongoing service coordination
  • Outreach and partnerships with community organizations and local Veteran service providers
  • Education and guidance on tenant rights, housing resources, and long-term housing stability

Position Summary: The Client Support & Service Coordinator – HMII provides client-centered case management and support services to individuals and families served by VOA Illinois. Using a trauma-informed approach, this role connects clients to community resources, monitors service plans, and promotes housing stability and overall well-being.
What You'll Do:
  • Provide intensive case management and crisis intervention
  • Conduct client intake, assessments, and evaluate psychological, physical, and service-related needs
  • Develop and monitor individualized service plans to support client stability, well-being, and progress toward goals
  • Conduct monthly home visits and community-based meetings to monitor client needs and service outcomes
  • Advocate for clients and connect them with community resources, service providers, and supportive programs
  • Serve as a liaison between clients, community agencies, and service providers to coordinate services and referrals
  • Educate clients individually or in groups on available services, application processes, and client rights
  • Utilize trauma-informed care, active listening, and conflict de-escalation strategies when supporting clients
  • Complete accurate and timely documentation in EchoVantage, HMIS, and other required databases
  • Collaborate with VOA Illinois staff, vendors, and community partners to ensure coordinated service delivery
  • Manage assigned caseloads with strong organization, time management, and responsible stewardship of agency resources
  • Exhibit cultural competence while serving clients of diverse backgrounds and commit to expanding knowledge and understanding of different communities and people groups
Work Environment & Schedule:
  • Full-time, in-office position averaging 40 hours per week
  • Eligible for at least one remote workday per week after Initial Training Period
  • Community-based travel required (Chicago metropolitan area)
  • Valid driver's license required
  • Ability to lift up to 50 lbs.
  • Physical demands include sedentary work, computer use, and frequent communication with clients, colleagues, and partners involved in client support.
  • Must be able to travel within the community, transport clients when necessary, and assist clients in and out of vehicles
Qualifications
Required:
  • Bachelor's degree in Human Services, Social Work, Psychology, or related field
  • 2+ years of experience in case management, human services, or working with vulnerable populations
  • Experience with trauma-informed care and leveraging community resources
  • Strong communication, collaboration, and de-escalation skills
  • Proficiency in Microsoft Office and documentation systems
Preferred:
  • Familiarity with local social service resources
  • Experience in advocacy, intake, or care coordination
  • Experience supporting veterans and military-connected individuals
