About This Position
Community Health Worker

Here's What You Need
- H.S. Diploma or equivalent is required. Successful completion of a Community Health Worker formal training program such as from a community college or other education institution is preferred.
- Six (6) months customer service experience either face-to-face or by phone is required, bi-lingual a plus.
- Experience working with pregnant and postpartum moms
- Familiarity with prenatal care, postpartum care, Medicaid, uninsured, or underinsured populations.
- BCLS - Basic Cardiac Life Support is required within 6 months of hire.
- Six (6) months of public health experience preferred
- Community health worker certification with the state of Texas is preferred.

What You Will Do
- Provide Client Support:
- Responsible for establishing trusting relationships with patients and families.
- Assist patients with personal goal setting and care plans.
- Supports clients with meeting personal goals including but not limited to attending appointments, providing support for scheduling meetings/appointments with agencies and resource providers. Efforts will support development of self-management skills that improve health and well-being.
- Collaborates with Senior Director of Faith/Community Health, regional Community Health Improvement initiatives and Community Health Ministry Strategy to implement Community Health Improvement activities:
- Participate in program development, planning and evaluation.
- Develop and effectively manage program timelines to ensure timely completion of program deliverables.
