LPN - Home Health & Hospice at Chadron Community Hospital, Corp. – Chadron, Nebraska

Monday-Friday, choice of 2 to 5 days a week. Call required every fifth weekend; number of weekday calls are based on hours worked. Full-time 30-40 hours/week.

Benefited position after 60-days waiting period.

Position Summary:

Works under the direction of the Home Health Clinical Manager and the Hospice Director with supervision from the Registered Nurses. The LPN provides direct nursing care, personal care and educating family/caretaker to assume care of patient. Coordinates with professional Home Health & Hospice staff and works under the general direction of a registered nurse. Utilizes the Electronic Medical Records.

Vehicles are provided most days, but some days will require use of own vehicle with mileage reimbursed. A good driving record is required.

Ideal candidate must be thoughtful, compassionate, professional and possess a variety of clinical skills. Position requires effective communication, attention to detail and proficiency with electric health records.

Educational Requirements & Experience:

• Associate Degree in Nursing from an accredited school of nursing.
• Licensed in the State of Nebraska without restriction.
• Experience preferred but not required.
