Key Responsibilities:

• Receive and process home health referrals via phone, fax, email, and hand delivery
  
• Verify insurance benefits and obtain authorizations from payers
  
• Accurately input patient data and visit details into the EMR and mileage tracking system
  
• Coordinate with the clinical team to schedule admissions and follow-up care
  
• Contact patients to confirm start-of-care dates and assign staff accordingly
  
• Track and update the daily admissions list, maintaining clear documentation
  
• Ensure timely communication with physicians for order sign-off and Face-to-Face documentation
  
• Collaborate across departments to support high-quality patient transitions

What We’re Looking For:

• 1+ year of experience in healthcare scheduling or referral coordination preferred
  
• Bilingual and/or experience working with diverse populations is a plus
  
• Calm under pressure and highly detail-oriented
  
• Familiar with EMR systems and Microsoft Office Suite
  
• Excellent verbal and written communication skills
  
• Comfortable working independently and as part of a dynamic team
  
• Reliable transportation and ability to travel locally if needed
  
• Enthusiastic about learning and growing into leadership opportunities
