POSITION SUMMARY: (WHAT, HOW, WHY)
Goodwill’s Rapid Re-housing program provides intervention to help individuals/families that are experiencing homelessness quickly move out of shelter or off the streets into permanent housing as quickly as possible. Rapid Re-housing provides shot term rental assistance and case management services designed to assist people experiencing homelessness to sustainable permanent housing. Responsible for: providing multiple case management services to all assigned participants. Direct service provision includes intakes, assessments, contracts for housing expectations, and acquisition for all program participants; independent living skills training as needed; and service strategy preparation. In addition, attends Housing-related meetings and client case consultations.
MINIMUM REQUIREMENTS:
EDUCATION: Bachelor’s degree in social services or related field. Licenses Social Worker (LSW) preferred.
EXPERIENCE: Minimum 1 year paid experience in Social Services.
KNOWLEDGE: Knowledge of case management techniques; familiarity with special needs of marginalized populations; knowledge of local community services; ability to communicate effectively (oral and written); knowledge and experience with computer software such as Microsoft Office Suite preferred. Knowledge of homelessness, housing-first approach, client engagement, and/or community resources, preferred. Ability to work independently.
ESSENTIAL DUTIES:
- All intake, assessment, and enrollment activities for the project.
- Conducts case management for assigned participants, including an Independent Living Skills need assessment that includes a housing history, debt vs. income, transportation mode and spending needs, budget and buying skills, time management, employment retention skills, and childcare needs.
- Educate and train participants in assessment areas that are possible deficits, community services, and eligibility for those services, and aid in accessing needed services. When necessary, will transport clients to other service provider agencies and assist with the referring of all necessary services.
- Maintain at minimum monthly follow-up following housing/placement until exited from the program; prepare and submit monthly computerized reports to supervisor.
Maintain close communication with all Housing program staff.
Maintain close communication with shelter/outreach staff to ensure a successful housing match under 30 days.
Identifies and problem-solves housing issues with landlords and the housing team to support housing retention.
Complete monthly home and office visits with clients, identifying and addressing barriers to prevent future episodes of homelessness.
- Effectively coordinates and communicates with internal/external service providers to support client care.
PERIODICAL DUTIES:
- Maintains client file/database of the Housing program.
- Prepares client folders to ensure all required forms are in each packet.
3. Records client data for grant reporting and Rehab Department Census and ensures that all client files are in order prior to client starting activities.
4. Prepares monthly activities report for supervisor.
OCCASIONAL DUTIES:
1. Works with other program staff as assigned.
2. Takes incoming phone calls and routes to appropriate office staff.
3. Other relevant and necessary duties as assigned by the supervisor.WORKING CONDITIONS: (SITTING, STANDING, WALKING, OVERTIME, ETC.)
- Sedentary/sitting/local travel.
- Multi-task orientation.
- Must be available for work during normal and peak customer service hours (generally Monday through Friday, daytime hours).
- Must have independent and reliable transportation for extensive local travel.
