Essential Job Functions/Responsibilities:
• Conduct assessments, treatment planning, evaluation of the response to care and readjustment of the treatment plan as needed
• Plan and provide community education to patients, families and peers
• Completes accurate and timely documentation of patient care treatments in EHR (i.e., HomecareHomeBase)
• Determine a PT treatment plan

Service Coverage Area:
• Corona and surrounding cities.

Position Qualifications:
• CA Physical Therapy License
• 2 years of physical therapy work experience
• American Heart Association Basic Life Support (BLS) certification

Job Type: Part-time, Per Visit

Pay: $56.00 - $140.00 per visit
