As the Community Health Manager, you’ll lead the strategy, expansion, and execution of Community Health programming across TAFB’s full-service area. You’ll champion the Ready to Nourish™ initiative, strengthen healthcare and community partnerships, and guide a team working to ensure that nutritious food is accessible, prioritized, and integrated into how our communities experience health.

Where YOU make the impact…

· Lead the strategy and expansion of Community Health Programming and the Ready to Nourish™ initiative across TAFB’s service area—growing a network of partners committed to nutrition-focused, client-choice environments.

· Build and steward strong relationships with partner agencies, healthcare organizations, and community leaders to advance food is medicine solutions and improve health outcomes.

· Guide partners through the Ready to Nourish™ Certification process, including assessments, training, nutrition policy adoption, and implementation of evidence-based strategies such as SWAP/HER nudges and environmental changes.

· Develop and advance a framework that demonstrates how access to nutritious and medically appropriate food improves health outcomes and reduces healthcare costs.

· Ensure successful execution of grants by aligning deliverables, managing timelines, and maintaining accountability for outcomes and budget considerations.

· Use data to drive decisions—track program performance, measure impact, and translate outcomes into compelling narratives for stakeholders, funders, and partners.

· Collaborate cross-functionally with internal teams (Marketing, Agency Services, Operations, Community Resources, Garden & Farms, Community Kitchen) to ensure seamless program delivery and partner success.

· Represent TAFB in local and state community health networks, staying current on food safety, public health trends, and behavior change strategies.

· Supervise and develop team members, including program staff and interns—setting goals, coaching performance, and fostering a culture of high performance and continuous improvement.

What you need to succeed!

· You’re a Registered Dietitian (RD/RDN) with a strong leadership presence and 5+ years of experience in community health, healthcare partnerships, nutrition programming, or related fields, including experience leading projects or teams.

· You have a proven ability to build relationships, influence decision-makers, and move initiatives forward across diverse stakeholders.

· You think strategically but execute practically—you can balance big-picture vision with day-to-day program delivery.

· You’re comfortable using data and metrics to guide decisions, demonstrate impact, and communicate results.

· You’re an exceptional communicator, with confidence in public speaking, training, and presenting to varied audiences.

· You lead with initiative and adaptability—you’re a problem solver who takes ownership and isn’t afraid to take smart risks.

· You have experience managing multiple priorities, grants, and deliverables in a dynamic environment.

· Bilingual Spanish is a plus.

· Most importantly, you are driven by purpose and committed to improving the health and well-being of the communities we serve.

· And yes—you’ll step in where needed. Leadership here is hands-on.

Requirements:

A day in the life…

· You’ll split your time between TAFB locations, partner sites, and the community. You might spend one day meeting with a healthcare partner, another reviewing program data, and another supporting implementation in the field.

· You’ll be active—standing, walking, and occasionally lifting up to 50 pounds.

· Travel across the service area is expected; reliable transportation, a valid driver’s license, and proof of insurance are required. We offer mileage reimbursement.

· Flexibility is key—occasional evenings and weekends are part of the work.
