What You Will Do:

As the Street Outreach Medical Assistant, you will provide field-based clinical support, harm reduction services, and care coordination to individuals experiencing homelessness. Reporting to the Outreach Program Manager, you will play a key role in bridging the gap between the healthcare system and unsheltered populations through direct outreach and engagement.


Your responsibilities include:

Field-Based Clinical Support:

Provide “backpack medicine” services in community settings, including taking vital signs, monitoring blood glucose, and performing basic wound care under the direction of an APRN. Assist with field-based exams and procedures in non-traditional environments. Support harm reduction efforts, including Naloxone (Narcan) training and distribution.

Medication Support and Management:
Assist with medication reconciliation to maintain accurate medication records during outreach encounters. Organize medications into portable, weather-resistant packaging to support adherence in outdoor environments. Maintain inventory of mobile medication supplies, ensuring medications are properly stored and within compliance guidelines.

Medication Observation and Education:
Support directly observed therapy (DOT) for high-priority treatments to promote adherence. Provide clear, low-literacy education on medication usage, timing, and side effects tailored to individuals living unsheltered. Monitor and follow up on reported side effects or complications.

Outreach and Engagement:
Conduct proactive street outreach to identify and engage individuals in need of care. Build long-term, trusting relationships through consistent presence and a non-judgmental, trauma-informed approach. Utilize de-escalation techniques in complex social interactions.

Care Coordination and Navigation:
Coordinate referrals and “warm hand-offs” to healthcare providers, shelters, and substance use treatment programs. Assist clients with navigating the healthcare system, including scheduling appointments and arranging transportation.

Logistics and Supply Management:
Maintain inventory, cleanliness, and sterilization of mobile medical kits and outreach supplies. Ensure readiness of equipment for daily field operations.

Documentation and Reporting:
Complete timely and accurate documentation of outreach encounters in Electronic Health Records (EHR) systems using mobile devices when available. Track services and supply distribution to support grant reporting and population health initiatives.

Safety and Medication Storage Support:
Assist in coordinating safe storage solutions for client medications when applicable. Track refill timelines and collaborate with pharmacies and care teams to ensure continuity of care.

Requirements:

Certification: Active Medical Assistant Certification (CMA or RMA) and BLS/CPR Certification


Experience: Minimum of 1–2 years of clinical experience. Preference given to candidates with experience in community health, emergency medicine, or behavioral health


Knowledge: Familiarity with Harm Reduction principles and the Social Determinants of Health (SDOH)


Driver’s License: Valid driver’s license with a clean driving record required for operating mobile units


Professional Competencies: High emotional intelligence, cultural humility, and the ability to remain calm in unpredictable environments
