Discharge Expediter at University Health System- San Antonio – San Antonio, Texas

POSITION SUMMARY/RESPONSIBILITIES

In collaboration with case managers, physicians, staff, community health resources and payers, is accountable for assisting Social Workers and Case Managers with coordination of care secondary to patient/family psychosocial/socioeconomic needs across the care continuum. Ensures timely care, compliance with regulatory requirements/standards and effective/efficient use of hospital resources. Completes established competencies for the position within designated introductory period.

EDUCATION/EXPERIENCE

High school diploma required. Associates degree preferred. Six months experience in a health care environment required. Preferred experience in HMO, hospital, home health or any clinical setting involved in discharge planning
