Compensation: $20-22/hr

Position Summary:
The Intake Specialist facilitates the referral and enrollment process at Community Living Alliance for all services offered.

Primary Duties:
- Conduct initial screening to identify available CLA services and supports to potential clients interested in CLA.
- Complete enrollment tasks and support referral action steps with internal and external partners to support onboarding for incoming clients.
- Coordinate with Program Managers to assist with identifying capacity and quality client enrollment experiences.
- Maintains accurate databases of referral, intake and enrollment data and provides regular reports related to maintained data.
- Attend community events, as needed.
Education/Experience and Requirements:
Required
- High school diploma combined with two (2) years of relevant work experience
- Two (2) years experience with long term care supports and services, program intake, and enrollment.
- 18 Years or Older
- Pass a WI Caregiver Background Check
- Work around environmental hazards such as odors, second-hand smoke, animals, cleaning supplies, adverse living conditions, etc.
- Capable of lifting/moving/pushing/pulling 35 pounds
- Must own or have access to reliable transportation to commute to the CLA office, client homes, vendor locations, community partner offices, and other relevant locations to complete essential functions of the position. Must possess a valid driver’s license, current WI auto insurance.
- Authorized to Work in the United States
- Bachelor’s degree in Human Services or related course of study, preferred.
- Two (2) years experience in customer service, public communications, community involvement, or related.
