About This Position
Job Title: Speech Therapist, PRN
Position Summary
- The Home Health Field Speech Therapist provides skilled speech-language pathology services in the patient’s home to improve communication, cognition, and swallowing functioning accordance with the physician’s plan of care.
Key Responsibilities
- Patient Evaluation: Performs comprehensive speech, language, cognitive, and swallowing assessments to determine impairments and therapy needs.
- Care Plan Development: Designs and updates individualized treatment plans based on assessment findings, physician orders, and patient goals.
- Therapy Interventions: Provides targeted therapy to address speech clarity, language skills, cognitive function, voice quality, and swallowing safety.
- Patient & Caregiver Education: Teaches strategies, exercises, and compensatory techniques to patients and caregivers to support communication and safe swallowing at home.
- Progress Monitoring: Tracks and documents patient progress, adjusting treatment plans as needed and communicating changes to the care team.
- Collaboration with Care Team: Coordinates with nurses, physical therapists, occupational therapists, aides, and physicians to ensure comprehensive, integrated care.
- Compliance & Documentation: Completes accurate, timely documentation in accordance with Medicare, Medicaid, and agency guidelines.
- Safety & Prevention: Implements swallowing safety protocols and communication strategies to reduce risks and enhance quality of life.
