Quality Improvement Coordinator

Salary Range: $30.40 - $41.91

Pay rates are determined based on experience and internal equity.

Position Summary:

  • Works under the general supervision of the Director of Quality Assurance/RM/IC.
  • Provide computer support in creating graphs and visuals for various Performance Improvement reports.
  • Pulls data reports from various databases such as Nuance, Press Gainey, AHA Survey, Quality Net, NHSN-SAMS, OSHPD, Q-Centrix, and others.
  • Perform data uploads, data validation, run reports and provide data maintenance for Get with the Guidelines,
  • Assist with data to day operations within quality.
  • Coordinate the collection and reporting of the Core Measures and other duties as assigned.
  • Knowledge of CMS,CDPH, Title 22, and CIHQ regulatory agencies.
  • Provide support for patient satisfaction and patient experience strategic goals.
  • Works well with supervisor, co-workers, patients, family members and visitors.

Position Duties/Responsibility:

  • Performs all assigned tasks in an efficient and safe manner.
  • Responds appropriately and timely to telephone and verbal requests.
  • Works well under stress or tight deadlines. Remains flexible to changing systems and processes.
  • Collects numerous data elements from individual records and abstracts clinical data on required elements from patient care records onto an established form following complex instructions on qualifiers for data elements to extract and exclusions; identifies outlier values or unusual data encountered in the abstraction process for additional review; applies an in-depth understanding of the diverse ways/places information may be found in a record to the process; understands and interprets clinically-oriented language, clinical 'jargon' and medical abbreviations.
  • Enters abstracted data into a web-based database for electronic transmission to third-party vendors for subsequent transmission to multiple Regulatory Agencies.
  • Anticipates potential areas of concern/interest and assists with/initiates tracking procedures; organizes data and develops/assists in development of reports that display compliance variations from regulatory standards; maintains ongoing variance reports and updates managers/interested parties when significant trends appear in the data; works with providers to interpret data trends.
  • Participates in cross-functional quality assurance audits of abstracted data to identify errors, outliers, etc. reviews error reports identifying data outside specified parameters; notifies source department to review or identifies as an appropriate exception.
  • Participates in data analysis by sorting data into appropriate categories.
  • May attend committee meetings to present and discuss data and identified trends.
  • Responsible for patient rounding to ensure that all patient needs are addressed and handles patient and/or family complaints in a nonjudgmental & compassionate manner. Escalates concerns to appropriate director in a timely manner. Enters the complaint into the appropriate database.
  • Active participation with BETA trainings and education to support validation.
  • Ability to log in and assign Patient Safety Events and Feedback Reporting to responsible leaders with all necessary information in a timely manner.
  • Prepares Mortality and transfusion information for review as required by regulation.
  • Prepares the Patient Safety Organization Report for submission.
  • Coordinate and pull data for physician peer review and ongoing physician performance evaluations, as well as assist with data for the Nursing team, Education, quality and administrative team. Performs related duties as required.

Essential Functions:

  • Must have the ability to answer the telephone, direct calls and interact professionally with co-workers and visitors.
  • Excel, and word proficiency. Ability to work in one drive, and SharePoint.
  • Must have the ability to sit for extended periods of time.
  • Must have the ability to retrieve charts for QAPI, Risk Management, as directed.
  • Prepares routine letters and memoranda for Director as requested.
  • Must have a background that includes knowledge & ability to use medical terminology comfortably.
  • Has positive personal interactions with staff, physicians, patients/residents and visitors.
  • Moderate understanding and knowledge of clinical regulatory and outcomes measures processes and ability to apply this knowledge effectively to perform job duties.

Position Qualifications:

  • Education: Associates Degree or equivalent capability through clinical experience. Bachelor’s degree in health-related field, preferred.
  • Experience: One year previous clinical/hospital experience, required.
  • Certifications: None required, HACP, CPHQ or other related certifications preferred.
  • Skills/Ability: Excellent verbal skills required. Exhibits the ability to organize multiple assignments and follow through with accuracy. Exercises good judgment under stress. Demonstrates maturity, initiative, emotional stability, tact and poise.
  • Ability to handle multiple tasks with interruptions, works independently, follows instructions and maintains a high level of professional conduct.
