Wages

$21.63 – 26.44 per hour

Job Summary

DARTS is hiring a Healthy Aging Specialist to connect people in contracted senior living communities to services and partnerships that improve their quality of life, help them age well, and enable them to engage in their community.

Responsibilities

  • Provide consultation on aging services to building residents.
  • Respond to requests of property manager to contact a resident about specific issues.
  • Assess service needs of residents and link them to the appropriate providers and community resources.
  • Maintain client records.
  • Track and produce data as requested and write assessment summaries.
  • Facilitate group respite for people living with memory loss.

What You Bring

  • Bachelor's degree in social work or equivalent experience preferred.
  • Spanish speaking proficiency is a plus.
  • Previous experience working with older adults.
  • Strong organizational and time management skills.
  • Ability to develop rapport with older adults who may have mental health issues and other life challenges.
  • Ability to work independently.
  • Your own reliable transportation.
  • Ability to maintain professional boundaries, confidentiality and strong problem-solving and customer service skills.
  • Ability to lead and develop group activities for individuals with memory loss.
  • Valid driver’s license and the ability to meet DARTS’s driving record requirements, or access to reliable transportation. Please note that if the license was not issued in the state of Minnesota, you have 60 days after becoming a Minnesota resident in which to apply for your Minnesota driver's license or permit (not applicable for new hires living out of state).
