Licensed Practical Nurse (LPN) Floater

Full Time- Mercer County

Job Summary:

The Licensed Practical Nurse provides direct assistance with daily dispensing and delivery of medications and assists with referrals to medical and community providers. The LPN works closely with medical providers, case managers, and clinical staff to ensure continuity of care, monitor client health, and support integrated treatment plans. The LPN performs basic data collection such as vital signs, weight, and symptom checks and responds to the signs and symptoms of illness or injury. The LPN will conduct medication counts, attend trainings, complete prior authorizations, and provide health education to consumers.

Education and Experience:

• Licensed Practical Nurse (LPN) with an active license to practice in West Virginia is required.
• Experience working with behavioral health consumers is preferred.
• Cross training to other programs will be expected.

The schedule will include 8 and 12-hour shifts. (Depending on program needs)
