About This Position
Oak Tree Child & Family Services is seeking a compassionate and organized Registered Nurse (RN) Case Manager to support children, adolescents, adults, and families receiving outpatient mental health services. The RN Case Manager plays a vital role in coordinating medical and behavioral health care, supporting psychiatric medication management, and ensuring continuity of services across providers, schools, community agencies, and families. This position supports Oak Tree’s mission of providing collaborative, trauma-informed, and family-centered care.
This is a hybrid-remote position, with some hours remote and some at Oak Tree Child and Family Services (511 Madison Ave in Fort Atkinson WI). A resume and cover letter are required for this position.
Requirements:Care Coordination & Family Support
- Conduct nursing assessments and health screenings as needed.
- Develop and monitor individualized care coordination plans in collaboration with therapists and psychiatric providers.
- Serve as liaison between Oak Tree providers, primary care physicians, psychiatrists, schools, and community agencies.
- Support families in navigating medical systems, specialty referrals, and community resources.
- Assist with discharge planning and transitions of care.
Medication Management Support
- Collaborate with psychiatric providers regarding medication monitoring and follow-up.
- Provide education to children, adolescents, and caregivers regarding prescribed psychotropic medications, including purpose, dosing, side effects, and adherence.
- Monitor for medication compliance concerns and potential adverse reactions.
- Communicate clinical concerns promptly and effectively to prescribing providers.
Crisis & Safety Support
- Participate in safety planning and crisis response planning.
- Provide clinical nursing support during periods of client de-escalation or heightened emotional distress (within outpatient scope).
- Coordinate higher levels of care when necessary (hospitalization, intensive outpatient, etc.).
Documentation & Compliance
- Maintain accurate and timely documentation within the electronic health record (EHR).
- Ensure documentation meets Medicaid, commercial insurance, and regulatory requirements.
- Assist with prior authorizations and insurance coordination when applicable.
- Maintain HIPAA compliance and uphold Oak Tree confidentiality standards.
Team Collaboration
- Participate in multidisciplinary case consultations and team meetings.
- Contribute clinical insight to treatment planning.
- Support Oak Tree’s culture of collaboration, professionalism, and ethical practice.
Qualifications
Required
- Current Registered Nurse (RN) license in good standing.
- Minimum of 2 years nursing experience.
- Experience working with children, adolescents, adults, or families.
- Knowledge of psychiatric diagnoses and psychotropic medications.
- Strong organizational, communication, and care coordination skills.
- Ability to work independently while collaborating within a team environment.
Preferred
- Experience in outpatient behavioral health or community mental health.
- Case management experience.
- Familiarity with Medicaid and commercial insurance processes.
- Experience in trauma-informed care.
