About This Position

We are seeking an experienced, clinically strong Pharmacist for Care Management to serve as an embedded medication expert within our care management team. This role focuses primarily on medication review, reconciliation, and consultation with care coordinators to optimize therapeutic outcomes, improve safety, and manage specialty drug utilization. This is a collaborative, team-based position with limited direct patient communication. The pharmacist will function as a clinical consultant to care coordinators and providers while supporting onboarding efforts for new specialists to understand the value of an embedded pharmacist model.

Key Responsibilities
  • Conduct comprehensive medication reviews.
  • Perform detailed medication reconciliation.
  • Provide medication-related cost and therapy estimates to assist care. coordinators.
  • Deliver clinical consultation to the care management team.
  • Provide in-depth analysis of:
    • Drug efficacy
    • Side effects
    • Drug interactions
    • Therapy optimization
    • Disease-state management
  • Support utilization and management of specialty medications.
  • Identify opportunities for improved medication adherence and cost savings.
  • Serve as a dedicated clinical resource for care coordinators.
  • Participate in interdisciplinary care management meetings.
  • Provide evidence-based recommendations to improve patient outcomes.
  • Assist in onboarding new providers/specialists by articulating the value of the embedded pharmacist program.
  • Support growth of the embedded pharmacist model.
  • Contribute to clinical workflow optimization.
  • Track and report measurable outcomes tied to medication management.
  • Participate in performance initiatives tied to care management metrics.

Qualifications
  • Doctor of Pharmacy (PharmD)
  • Active and unrestricted Pharmacist license
  • Multiple years of clinical pharmacy experience
  • Strong knowledge of disease states and pharmacotherapy
  • Experience with specialty medications
  • Strong analytical and consultation skills
  • CSP (Certified Specialty Pharmacist)
  • SPC certification
  • Experience in care management or population health (not mandatory)
  • Ability to effectively communicate value of services to providers
  • Business acumen and comfort in growth-oriented healthcare models
