About This Position
Position and Scope of the Opportunity
The social worker position reports directly to the COO. The social worker position helps lead the planning and development of objectives and initiatives as it relates to care coordination. Primary responsibilities include assessing, admission, and case management of potential customers and families, and existing clients and families. The social worker will follow up with employees, clients, referral sources, and other health care organizations, to coordinate care and ensure the delivery of the highest quality of services to all of the clients and families that we serve. The social worker will also actively engage the community working to develop and enhance partnerships with referral sources.

Duties and Responsibilities
Administrative
Provide excellent customer service through verbal and written communication including answering phone calls and responding to emails in a timely manner.
Manage caregiver credentials.
Create case related documents, folders, and care plan books for clients.
Review timesheets, hours worked and maintains accurate billing/payroll records.
Care Coordination
Complete intake and assessment process associated with caseload.
Execute Weekly Care Plan Meetings with Sunland Home Care office Staff and be familiar with Sunland Home Care clients and potential client needs.
Maintain Positive Client and Family Relationships.
Resolve client and caregiver concerns associated with assigned caseload.
Complete on-site client visits with clients and caregivers per scheduling requirements.
Monitor and assess the quality of services being provided for each assigned case.
Participate in daily care plan meetings ensuring client needs are being met from both a scheduling and plan of care standpoint.
Scheduling
Oversee client/caregiver relationships and care coordination of all staffing needs associated with caseload.
Oversee employee availability and work schedules and handle employee requests for information or time off and other related duties.
Serve as the point of contact for assigned caseload coordinating with clients and caregivers about schedules, conditions and any changes.
Participates in after-hours on-call rotation, including assigned weekends and holidays.
Employee Engagement
Maintain Positive Employee Relationships with existing employees and potential employees.
Assist with leading new hire orientations, training, and staff development projects.
Participates in the screening and hiring process for direct care workers.
Community Outreach
Represent Sunland Home Care at Community Events, Marketing Opportunities, and Conferences.
Develops and maintains partnerships with referral sources.
Minimum Qualifications
Bachelor's degree in Social Work or related field
2 years of care coordination / case management experience.
2 years experience in the home care, assisted living, skilled nursing, or related healthcare fields.
Must have a valid driver’s license, good driving record and current auto insurance, with applicable proof of certification and / or licensure.
Preferred Qualifications
Master's degree in Social Work or related field.
Experience with hospice care.
Physical Requirements
Prolonged periods sitting at a desk and working in a computer
Must be able to independently perform community and home visits.
Must be able to lift up to 25 pounds at times.
Work Location and Environment
Hybrid position working in the office, community and remotely.
