Job Description:

St. Gabriel's Hospice and Palliative Care is seeking a compassionate and experienced RN Case Manager to join our dedicated team in Houston, Texas. As an RN Case Manager, you will play a crucial role in providing high-quality hospice and palliative care to our patients, ensuring they receive the best possible support during their end-of-life journey.

Key Responsibilities:

• Conduct comprehensive assessments of patients' needs, symptoms, and conditions.

• Develop and implement individualized care plans in collaboration with the interdisciplinary team.

• Provide skilled nursing care, including pain and symptom management, wound care, and medication administration.

• Educate patients and families about their conditions, treatment options, and care plans.

• Coordinate with physicians, social workers, and other healthcare professionals to ensure holistic and patient-centered care.

• Monitor and document patients' progress, adjusting care plans as necessary.

• Advocate for patients' and families' needs, ensuring they have access to necessary resources and support.

• Participate in team meetings and case conferences to discuss patient care and outcomes.

Qualifications:

• Current RN license in the state of Texas.

• Minimum of 2 years of experience in hospice, palliative care, or a related field.

• Strong clinical skills and knowledge of end-of-life care practices.

• Excellent communication and interpersonal skills.

• Ability to work independently and as part of an interdisciplinary team.

• Compassionate and empathetic approach to patient care.

• Valid driver's license and reliable transportation.
