Outpatient Counselor-SCI

POSITION SUMMARY

The Outpatient Counselor/Clinical Care Manager — SCI coordinates and provides counseling, pro-long term support, psychosocial support, and other services to people who are outpatients with spinal cord injuries after discharge from acute/rehabilitation care. Counselors/Clinical Care Mangers are patient advocates, team leaders/liaisons, clinical team members, liaisons with insurance and other funding resources and discharge planners.

Posting Closing Date: March 31, 2026

Rate of pay: $33.50 - $48.23 per hour based on experience

POSITION REQUIREMENTS
  • Licensure/Certification/Registrations: Current Colorado Driver's License.
  • Education: Master's Degree in the field of Social Work, Psychology, Counseling or related field required. Specialized training in rehabilitation or equivalent experience working with people who are spinal cord injured preferred.
  • Experience: Minimum of two (2) years experience preferred working with people who are spinal cord injured with an emphasis on coordinating services in the community for, with, or on behalf of patients.
  • Skills & Abilities: Ability to understand/manage the complexities presented by people with spinal cord injuries. Skilled counselor with the ability to work independently, organizing time effectively. Ability to identify patient issues/problems, develop plans/solutions and take appropriate action with minimal supervision. Skilled in the operations of the basic office equipment. Ability to effectively communicate both verbally and in writing. Must have excellent skills to maintain good interpersonal relationships. Ability to work with patients of varying age groups. Ability to analyze financial and medical information and to complete financial documents and medical necessity letters for, with, or on behalf of patients. Familiar with federal, state and local resource systems. Computer skills including Microsoft Office and EPIC (EMR).
ESSENTIAL FUNCTIONS
  • Complete needs assessment to determine services and structure needed to help patients achieve maximum independence.
  • Coordinate, monitor and evaluate options and services by providing quality, cost-effective interventions and outcomes required to meet the rehab patients' needs.
  • Function as a liaison, advocate, negotiator and educator for patients with SCI
  • Assist patients in problem-solving and coordination of services.
  • Assist patients regarding reintegration into the community after spinal cord injury.
  • Analyze financial, medical, and other information and complete financial and other documents for, with, and/or on behalf of patients/families.
  • Participate effectively as part of an interdisciplinary team to serve people with spinal cord injury.
  • Ensure accurate and timely documentation
