Social Worker- Care Management at Cookeville Regional Medical Center – Cookeville, Tennessee

The Social Worker is responsible for providing a wide range of social services in accordance with the hospital mission and objectives. Services provided by Social Service include assisting patients and their families in addressing social, emotional, and economic issues associated with illness or injury. Crisis intervention, discharge planning, continuity of care, financial assistance and referrals to community resources, while interacting with interdisciplinary team are some of the assigned duties. Other duties as assigned.

Education: Bachelor's or Master’s degree in Social Work required. Equivalent degree will be considered. At least one year social work experience, preferably in health care.
