As a Start of Care Specialist, you’ll be the first point of contact for newly enrolled Medicaid members—playing a key role in assessing needs, completing documentation, and building a foundation for long-term, person-centered care. You'll conduct in-home assessments, develop care plans, and ensure all enrollment materials are completed, setting the stage for successful ongoing care coordination.
What You’ll Do
- Conduct comprehensive, in-home assessments for newly enrolled Medicaid members
- Develop individualized, SMART care plans that reflect each member’s needs and goals
- Collaborate with care teams, providers, and collateral contacts to ensure holistic planning
- Input member data into the EHR with accuracy and timeliness
- Track and meet enrollment deadlines, including completion of assessments and documentation within the required 56-day window
- Coordinate and review enrollment packets to ensure all consents and signatures are completed
- Ensure compliance with HIPAA and other privacy regulations
- Provide administrative support to care management team as needed
What You Bring
- Social Service Bachelor’s degree with 0-1 years of relevant experience; or
- Non-Social Service Bachelor’s degree with 2+ years of relevant experience; or
- Associate’s degree with 3+ years of relevant experience.
Experience & Skills
- Prior experience in an administrative, healthcare, or care coordination support role
- Familiarity with Health Home, case management, or social service environments a plus
- Empathetic, person-centered approach to care
- Excellent documentation, organizational, and time management skills
- Proficient in Electronic Health Records and Microsoft Office
- Ability to work independently and travel throughout the community
- Bilingual abilities are highly desirable·

The pay range for this role is:
19 - 22 USD per hour(NY, NY)
