Wage Range: $111,592 - $178,485 per year
Posted wage ranges represent the entire range from minimum to maximum. For jobs with more than one level, the posted range reflects the minimum of the lowest level and the maximum of the highest level. Some positions also offer additional premiums based on shift, certifications or degrees. Job offers are determined based on a candidate's years of relevant experience, level of education and internal equity.
Job Summary:
Serves as a point of contact for the referring physicians, patients, and caregivers to provide resources and assistance with accessing clinical and supportive care services offered by EvergreenHealth and in the community. Assists with the comprehensive care of complex surgical patients and enhances the patient experience by interfacing with various patient care teams. Collaborates with the multidisciplinary teams to ensure patients receive integrated care for optimum outcomes. The Nurse Navigator serves as a patient-centered resource by providing support, education, and continuity of care for specific surgical patients. Facilitates and coordinates patient care to ensure all patients receive quality and comprehensive services. Functions on the multidisciplinary team as an advocate, educator, and counselor for surgical care patients.

Primary Duties:
- Coordinates and maintains patient care plans in accordance with established guidelines.
- Serves as an essential link between patients, care givers and all other care providers.
- Assists patients in understanding their diagnosis, treatment options, and the resources available.
- Follows up with patients as directed and collaborates with surgical care providers to develop and maintain care plans appropriate to diagnosis and treatment.
- Monitors patients throughout the care continuum, including patient chart reviews to determine appropriate pre-op testing and clearances and inpatient admissions. Collaborates with inpatient, outpatient, and community care management resources.
- Facilitates consultation appointments and educates patients and family about appropriate supportive care resources, including social workers, dieticians, rehabilitation, etc.
- Ensures timely clinician review by gathering patient diagnostic studies and collaborating with patient care teams.
- Responsible for the outreach efforts to establish and maintain positive working relationships with key customers (physicians, mid-levels, office staff, diagnostic staff, nurses, and other care team members).
- Works with surgical care leadership on initiatives to expand access, education and services for surgical care patients.
- Collaborates with clinical leadership and appointed workgroup members to assist in building support for specific patient populations within the surgical care clinic.
- Monitors, updates, and collaborates on new and established reporting for specific patient populations within surgical care.
- Collaborates with clinical leadership and administrators to establish patient population quality metrics and monitors for any significant issues or concerns.
- Provides reporting and metric updates to clinic and administrative leaders on a regular cadence.
- Performs other duties as assigned.
License, Certification, Education or Experience:
REQUIRED for the position:
- Bachelors in nursing (BSN)
- Licensed as a Registered Nurse in the State of Washington
- 3 years of experience in an ambulatory clinical setting or care management experience
- Current Healthcare Provider BLS American Heart Association by start date
DESIRED for the position:
- Masters Degree
