About This Position
Summary
The Admissions Specialist assists the Admissions RN’s/SCM team by advising callers of our services and the admission Requirements and procedures. In addition, the Admissions Specialist communicates with referral sources to request the necessary information needed for the admission and/or consultation. Ensures information is entered into the electronic medical record, schedules appointments and completes the proper follow-up.
Physical Demands
Spend long periods of time sitting and using office equipment and computers.
Works frequently on repetitive tasks such as keyboarding.
Communicates via telephone, may use headphones and earpiece.
Must be able to communicate with and listen to callers.
Occasionally lifts 5-10 pounds
Work Environment
Work is performed in a typical office environment.
Essential Duties and Responsibilities
- Receives referrals/requests for information from referral sources and community members.
- Provides accurate information regarding our services & funding in a professional and sensitive manner.
- Utilizes established screening process demonstrating compliance with Hospice Alliance admission criteria.
- Coordinates discharge, delivery of equipment, supplies required to initiate care in the environment of patient choice.
- Coordinates and collaborates with admission RN’s/SCM team on scheduling, assignments and admission process per agency protocols.
- Obtains medical records to assist in the eligibility for Hospice Alliance services.
- Enter patient information into the EMR and follow up with those referrals that are not yet determined to meet medical eligibility criteria or were discharged from services within established guidelines.
- Other duties as assigned.
Education and/or Experience Requirements
- High school diploma or GED or equivalent years of experience required Associates degree or certificate in related field preferred.
- 2 years’ experience in admissions and/or medical insurance preferred.
- Proficiency with Microsoft office, computer skills and, familiarity with an electronic medical records is required.
- -Excellent communication skills and phone etiquette.
- Good organization skills.
- Detail orientated.
