Arizona Home Care is seeking a dedicated Intake Coordinator to join our in-office care coordination team, supporting both home health and home infusion services. In this role, you’ll play a key behind-the-scenes role in ensuring patients receive the care they need—accurately, efficiently, and on time.

Schedule:
  • Full-time, in-office position
  • Wednesday–Sunday or Friday–Tuesday
  • 8:30 AM – 5:00 PM
  • Holidays worked as needed based on department scheduling requirements

Position Summary

The Intake Coordinator is responsible for the accurate and timely processing of referrals for General Home Care and Patient Care Infusion services. This role serves as a key liaison between hospitals, physicians, referral sources, and internal clinical teams to ensure appropriate, compliant, and timely starts of care.

The Intake Coordinator demonstrates strong organizational and time-management skills, communicates proactively, and supports positive relationships with both internal and external customers.

Key Responsibilities
Referral Intake & Processing
  • Access and manage referrals through electronic referral portals and systems such as Strata, CarePort, Aidin, etc.
  • Accurately and timely process all incoming home health and home infusion referrals
  • Evaluate referral requests for appropriateness of services
  • Secure patient demographics, clinical information, and required documentation
Patient Setup & Data Entry
  • Accurately enter patient information into EMR systems (KanTime and CareTend)
  • Ensure all required documentation is complete and ready for intake processing
Insurance & Clinical Information Coordination
  • Obtain and review insurance coverage, authorizations, and payer requirements
  • Collect and organize clinical information including diagnoses, medications, lab results, procedures, and physician details
  • Coordinate with agency staff regarding patient progress, transitions, discharges, or readmissions
  • Assist with insurance verification and financial quotes as needed
Communication & Collaboration
  • Act as a liaison between patients, referral sources, physicians, facilities, and internal teams
  • Provide information regarding agency services, policies, and programs as needed
  • Work closely with community liaisons, clinical staff, pharmacy, and care coordination teams
  • Assist with training new intake staff as needed
Quality, Compliance & Professional Conduct
  • Maintain compliance with all state, federal, and local regulations and accrediting body standards
  • Demonstrate excellent customer service, professionalism, and advocacy
  • Promote a positive team environment
Requirements:
  • Experience in healthcare intake, scheduling, referrals, or care coordination
  • Knowledge of home health and/or home infusion regulations, including Medicare and Managed Care
  • Familiarity with electronic referral portals and EMR systems
  • Experience with KanTime and/or CareTend is preferred
  • High school diploma or equivalent
  • Ability to pass background check and OIG screening
  • Strong organizational, time-management, and multitasking skills
  • Excellent verbal and written communication skills
  • Ability to work effectively in a fast-paced, deadline-driven environment
  • Comfort learning and using multiple software systems
