About This Position
Job Overview:
The Case Manager facilitates treatment components of EISS. Provide direct practice in the areas of group work, consumer education, treatment planning and individual counseling as needed. Participate as a member of the multidisciplinary treatment team, performing intakes, assessments, initiating treatment interventions and crisis intervention. Perform clinical case management and treatment planning with consumers living with HIV/AIDS. All clinical documentation is completed with compliance in the requirements set by the Joint Commission on Accreditation of Health Care Organizations. Participates in staff training and continuing education. Takes an active role in Performance Improvement projects at the Ryan White Clinic. Is responsible for reporting identified safety issues such as hazardous environments (i.e. damaged floors/walls/ceiling tiles/unsecured areas) and medical errors, etc.
Qualifications:
Required:
- BA/BS Degree
- 3-5 years experience working with those living with HIV/AIDS and/or lived experience
- Valid NJ state Driver's license
- Possess an understanding of community level work and the importance of collaborating and coordinating with community based organizations
- Computer profeciency
Preferred:
- Fluency in Spanish

Essential Functions:
- Complete an intake with each PLWHA interested in case management, in which client/patient specific information, barriers to care and urgent needs are assessed
- Works with peers, team members to orient the client/patient to the organization with a site tour, overview of available resources, and introductions to treatment team, wrap around teams, etc, if applicable
- Meets with clients/patients to complete additional intake paperwork, authorizations, consents based upon desired services, etc.
- Provides client/patient with linkage to an insurance assister if needed, obtains required documentation from clients/patients and assists with applications for medication assistance or ADDP as needed
- Promotes client/patient access to internal agency services including, but not limited to: support groups, housing services, food programs, behavioral wellness, substance-use treatment, and medical care
- Performs other duties as assigned
