CHMC –Wellness Center-LMSW– Full Time- Day Shift – Requisition 9348

LMSW Pay Range: $30.60-$40.54*

Job Summary:

The Social Worker serves as a core member of the collaborative care team, functioning as the mental health care coordinator. In this role, you will be responsible for supporting the mental health care provided by primary care providers, often in consultation with a psychiatrist or other mental health specialists. You will also coordinate crucial patient referrals to clinically indicated services outside the primary care clinic, such as social services, mental health specialty care, and substance abuse treatment.

Qualifications

• Education/Licensure:

  • Master's degree in Social Work required (LMSW or greater).

  • Must be licensed by the state of New York.

• Certifications/Training:

  • Must maintain current BLS certification.

  • Must have completed coursework or training in the identification and reporting of child abuse and be a NYS mandated reporter.

• General Requirements:

  • Must possess good moral character.

  • Ability to perform physical requirements including bending, lifting, standing, and walking.
