About This Position
RiseWell Community Services is looking for a Licensed Practical Nurse for our ACT Team in our Clinical Department.
In this role, you will be providing management and administration of medications, in conjunction with the psychiatrist, NPP, and registered nurse as a member of a mobile, multidisciplinary treatment team for persons with severe and persistent mental illness. Ensure medical standards as per program regulations are being provided.
QUALIFICATIONS: Currently licensed as a Licensed Practical Nurse by the New York State Education Department. A valid and clean NY State driver's license. Minimum 1 year of experience in a treatment or rehabilitation setting preferred. Fingerprinting, criminal record check, and approval from NYS Office of Mental Health.
JOB FUNCTION:
Provides health services including medication management and education as a member of a multidisciplinary mobile support team for persons with severe and persistent mental illness in scattered site housing programs or working with clients preparing to transition into independent housing.
ESSENTIAL FUNCTIONS:
- Provides medication management services for all assigned clients. This includes, but not limited to supervision of medication, maintaining supply of medication, counting of medication, ordering and assessing medication holds.
- Update EHR within 24 hours of all medication changes and ensuring Medication Sheets are up to date at all times.
- Maintain organization of medication boxes in the community as needed.
- Communicate with RN daily regarding any medication management concerns.
- Completion of documentation for any contacts made in the EHR within 48 hours of occurrence.
- Completion of Physical Health Assessment for all assigned clients within 30 days of admission to program.
- Provide on-going education and counseling to assist clients who have increasing medical concerns as needed.
- Coordinate with outside providers including hospitals, nursing homes, psychiatric and medical treatment providers as needed.
- Will participate in inpatient discharge meetings and other collaborative meetings when necessary.
- Provide linkages to psychiatric and medical appointments as needed.
- Actively participate in weekly staff meetings and monthly high risk meetings.
- Complete all required trainings monthly as per the agency-wide training schedule.
- Actively participate in supervision a minimum of once a month with the RN and once a month with the Program Supervisor.
- Management of crisis situations under the direction of the Program Supervisor and document incidents as per agency policies/procedures.
- Comply with agency Compliance Plan and False Claims Policy by preventing, detecting and reporting any potential healthcare abuse and fraud. Report any potential non-compliance and comply with all other federal, state and local regulations applicable to this job description.
- Utilize all agency-based IT applications in a proficient manner including becoming proficient in the use of the IPad, collaborative documentation, review of emails at least twice per shift (at start and prior to close of the shift), as well as utilizing Outlook Calendar on a daily basis.
- Provide coverage and assistance for the RTS team as needed.
- Perform all other job duties as assigned.
