About the Role:
Sierra Home Health in Truth or Consequences is seeking a Physical Therapist Assistant (PTA) to join our compassionate and collaborative home health team. Key Responsibilities:

• Deliver in-home physical therapy services under the supervision of a licensed Physical Therapist
  
• Carry out treatment plans to help patients regain mobility, function, and independence
  
• Communicate progress and changes in patient condition to the supervising therapist
  
• Educate patients and caregivers on exercises and safety practices
  
• Document visits and progress notes accurately and on time
  
• Participate in care team meetings and interdisciplinary planning
  
• Uphold infection control and safety standards during all home visits
  
• Maintain professional, compassionate rapport with patients and families
  

What We’re Looking For:

• Current licensure as a Physical Therapist Assistant in New Mexico
  
• Previous experience in home health preferred, but not required
  
• Strong interpersonal and communication skills
  
• Confidence working independently in the field
  
• Adaptability to changing schedules and home environments
  
• Commitment to quality, safety, and patient-centered care
  
• Reliable transportation and valid driver's license
