As a Team Coordinator, you keep the flow of care organized, accurate, and timely. From scheduling and communication to document management and supply coordination, you play a vital behind-the-scenes role in helping the team deliver compassionate, efficient care.

Your Role in the Mission

As a Team Coordinator, you serve as the administrative backbone of the interdisciplinary team. You manage schedules, organize communication, and ensure clinical documentation is accurate, timely, and accessible. You help prepare for meetings, maintain patient records, coordinate supplies and equipment, and respond quickly to needs as they arise.

Key Responsibilities
  • Support communication flow – Answer phones, triage messages, and ensure timely follow-up by routing communication to the appropriate team members.
  • Manage clinical documentation – Upload and distribute physician orders, history and physicals (H&Ps), and other clinical records in the EMR system to support seamless care.
  • Coordinate team meetings – Prepare agendas, distribute notes, and support facilitation of Interdisciplinary Team (IDT) meetings.
  • Maintain patient records – Assemble and update patient binders and ensure necessary materials are available for facility visits and team conferences.
  • Coordinate durable medical equipment (DME) – Facilitate ordering, delivery, repair, and pickup of DME to meet patient needs in the home or facility setting.
  • Notify pharmacy of key updates – Alert pharmacy to all patient admissions and deaths to support timely medication management and billing accuracy.
  • Manage supply logistics – Track, order, and distribute medical and office supplies to ensure field staff are equipped for care delivery.
  • Assist with administrative tasks – Support day-to-day operational needs, including file maintenance, tracking, and additional duties as assigned.
  • Adapt and respond – Remain flexible in supporting changing team and patient needs, with a willingness to adjust priorities throughout the day.
Skills & Qualifications
  • High school diploma or equivalent required; college degree preferred.
  • Excellent interpersonal and organizational skills with strong verbal and written communication.
  • Proven ability to prioritize tasks, make decisions, and solve problems in a fast-paced environment.
  • Comfortable working both independently and as part of a team.
  • Basic proficiency in computer systems; EMR experience preferred.
  • Valid driver’s license, reliable transportation, and auto liability insurance.
  • Must be able to read, write, and communicate clearly in English.
Where You’ll Work

In this role, you’ll work primarily from a local office or team hub, supporting field-based staff who deliver care across the community. You’ll collaborate daily with nurses, social workers, chaplains, physicians, and administrative leaders to ensure care coordination runs smoothly and efficiently.

This role may require occasional local travel, flexibility in scheduling, and the ability to support meetings or needs outside traditional business hours. You’ll need to be comfortable managing multiple responsibilities while maintaining peace, professionalism, and presence.
