APRN - HEART FAILURE - CARDIOLOGY at Saint Bernards Medical Center – JONESBORO, Arkansas

The HF Coordinator - APRN will work as part of the heart care team to help coordinate care in a timely fashion to improve patient outcomes. Coordinator will monitor and track suspicious imaging findings on routine and screening studies as well as follow all positive pathology results and work with physician team to ensure appropriate and timely follow up studies and treatment are received. HF Coordinator will be responsible for communicating with patients and following care throughout their treatment as well as providing a survivorship care plan upon graduation. The HF Coordinator will work closely with heart care team to ensure psychosocial needs are addressed and will assist with referrals to support services if needed.
