About the Role:

The Casemanager I - MH ONLY plays a critical role in supporting individuals with mental health needs by coordinating and facilitating access to appropriate services and resources. This position focuses on assessing client needs, developing individualized care plans, and monitoring progress to ensure effective treatment and support. The Casemanager acts as a liaison between clients, healthcare providers, and community resources to promote holistic well-being and recovery. By maintaining detailed documentation and advocating for client needs, the Casemanager helps to improve client outcomes and quality of life. This role requires a compassionate, organized, and proactive professional dedicated to empowering clients through comprehensive case management within the mental health field.

Minimum Qualifications:

  • High school diploma or equivalent; Associate’s degree in human services, psychology, social work, or related field preferred.
  • Experience working with individuals with mental health conditions, preferably in a case management or support role.
  • Basic knowledge of mental health disorders, treatment modalities, and community resources.
  • Strong organizational and communication skills.
  • Ability to maintain confidentiality and adhere to ethical standards in client care.

Preferred Qualifications:

  • Bachelor’s degree in social work, psychology, counseling, or a related field.
  • Certification or training in case management or mental health support services.
  • Experience with electronic health records (EHR) and case management software.
  • Familiarity with local, state, and federal mental health regulations and resources.
  • Bilingual abilities or experience working with diverse populations.

Responsibilities:

  • Conduct comprehensive assessments of clients’ mental health status and service needs.
  • Develop, implement, and regularly update individualized care plans in collaboration with clients and multidisciplinary teams.
  • Coordinate referrals and link clients to appropriate mental health services, community resources, and support systems.
  • Monitor client progress and adjust care plans as necessary to ensure effective treatment and support.
  • Maintain accurate and timely documentation of client interactions, service plans, and outcomes in compliance with organizational and regulatory standards.
  • Advocate on behalf of clients to ensure access to necessary services and support.
  • Participate in team meetings and collaborate with healthcare providers, social workers, and other stakeholders to optimize client care.

The Casemanager I utilizes strong interpersonal and communication skills daily to build trust and rapport with clients and collaborate effectively with healthcare providers and community partners. Organizational skills are essential for managing multiple cases, maintaining detailed records, and ensuring timely follow-up on client needs. Critical thinking and problem-solving skills enable the Casemanager to assess complex client situations and develop appropriate care plans. Knowledge of mental health conditions and available resources allows the Casemanager to make informed referrals and advocate effectively for clients. Additionally, proficiency with technology, including case management software and electronic documentation, supports efficient and accurate record-keeping and reporting.
