Center for Life Management (CLM) provides the area's most comprehensive array of outpatient mental health services for adults and elders. Our Integrated Care Program is a highly innovative approach to whole healthcare that encompasses the integration of treatment modalities for mental and physical health needs. The Integrated Care (IC) team utilizes a wrap-around care model that is necessary to support individuals with addressing the intersection of their complex mental and chronic physical health conditions.

The Onboarding Case Manager ensures a successful introduction to Case Management services at Center for Life Management to clients who have completed a recent intake and to newly hired case managers needing introduction, support, and training. This position identifies client transition needs and engages the individual in various interventions while they await permanent staff assignment. Interim services to be provided may include care planning, resource identification, benefit coordination, service monitoring, and community-based supportive counseling to assist clients with improving symptoms and behaviors that otherwise interfere with daily functioning and wellness goals. This position also provides training to staff in the provision of case management services and affiliated documentation. This is a full time, 40 hour per week position.

Major Functional Areas and Description of Duties

  • Provide case management services to assigned caseload to include:
    • Provide extensive outreach, case management, and community-based services to clients who are awaiting permanent case management assignment.
    • Provide individual assessment to create care plans that specify presenting problems, objectives, interventions, and desired outcomes.
    • Assist clients with applying for available benefits and maintaining adequate medical coverage to support consistent access to whole health care and treatment.
    • Provide outreach, advocacy, coordination, resource identification, linkage and monitoring of internal and external supports and services.
    • Provide Individual Therapeutic Behavioral Services (TBSI) to develop, apply, and reinforce coping strategies to reduce and/or ameliorate symptoms and behaviors that interfere with functioning and goal achievement.
    • Provide Crisis Intervention (CI) when clients experience acute exacerbation of symptoms to ensure their safety within the home and/or community.
    • Develop priority assignment of cases for disposition and articulate client needs effectively for proper case placement.
    • Work collaboratively with other treatment team members, natural supports, and external providers to ensure a wrap-around approach is provided for continuity of client care.
  • Provide training to identified staff in the provision of case management services and affiliated documentation through instruction, modeling and shadowing opportunities, to include:
    • How to conduct outreach phone calls and face-to-face meetings to engage clients in case management services.
    • How to assess individuals to determine eligibility for services, complete thorough care plans, and update treatment plans to achieve desired outcomes.
    • How to document case management contacts and services within electronic health record.
  • Function professionally and in a manner that protects the integrity, confidentiality and rights of all patients.
  • Execute all required services and timely documentation on each service delivered in accordance with agency policies and federal/state standards.
  • Maintain professional memberships, licenses, accreditation, certifications, etc.
  • Represent CLM in a professional and appropriate manner in all settings.
  • Perform all other duties as assigned by supervisor.

Physical Demands and Working Conditions

The work will be primarily community-based to include visiting clients in their homes and in their environment. Additionally, work will be performed in the office and remote via telehealth. Manual dexterity is needed to use computers and standard office equipment. The ability to sit for extended periods of time is required. Reliable internet for remote work is required. Reliable transportation, a clean driving record, and proof of auto insurance coverage is required for this role.

The Center for Life Management has partnered with New England College to offer a Master’s Degree! This is an exciting opportunity for CLM staff to earn a Master of Science in Clinical Mental Health Counseling (MS) degree in 3 years. Who’s Eligible and What’s Required:

  • Must be a regular, full-time CLM employee in good standing for 9 months or more.
  • Must hold a Bachelor’s Degree.
  • Must agree to a five (5) year commitment of full-time employment in good standing with the Center for Life Management (beginning from start date/year of Professional Development Program participation).
  • Participants must maintain a B average to continue participation in the program.

Requirements:

Qualifications and Skills

  • Bachelor’s degree preferred however an Associate’s degree along with a minimum of two years of direct service in a related field may qualify.
  • Ability to exercise sound clinical judgment in routine, urgent, and emergent situations.
  • Proficient computer skills, specifically with all Microsoft office applications.
  • Ability to document effectively and efficiently in course of clinical work.
  • Ability to navigate an electronic health record.
  • Ability and desire to work as part of a clinical team while also able to be self-directed and motivated with minimal supervision.
  • Ability to acquire and effectively practice new clinical skills as needed according to evolving agency needs/services.
  • Effective communication skills, organizational skills, and ability to multi-task.
